Personal Telephone Service
Start the process now and purchase the number of LPA’s you require and we will call you and take all of the information we need from you over the phone and your payment. We will then populate all of the forms for the Office of Public Guardian and then send you out the forms to sign off. You then return the forms to us in the prepaid business envelope and we send then off to the Office of Public Guardian after one last professional scrutiny.

After you have purchased the amount of LPA’s you need an account will be created for you which gives you immediate access to forms that you can populate with the information we need about you (donor) the people you trust (attorneys) and the person you are going to nominate as your certificate provider.
Once these forms are populated we will use that information to ensure that the submissions to the Office of Public Guardian meet the criteria for registration first time. We have been submitting and registering these forms for years and do not worry once your membership account is created you will have complete access to our support team who will ensure that all of the i’s are dotted and the t’s are crossed making this as easy as possible and stress free.
